RBC Sports Ministry Youth participates in numerous sporting activities throughout the San Antonio area.  If you are interested in participating, please fill out the registration form below.  An RBC sports representative will contact you. 

If the participant is a minor, this form must be completed by the parent or guardian only. IMPORTANT NOTE: ALL FIELDS REQUIRE INPUT-- PLEASE USE N/A FOR ANY FIELD THAT DOES NOT APPLY.

For activities that require a fee (see list below), please complete this registration form first and you will be redirected to the RBC E-Store (our Online Shopping Cart) where, you may pay your registration fee. If you prefer, you may also pay your registration fee at the church information booth.

Participant may only register for the sport/league during its active registration period. Registration fee for all sports is $30.00, except Basketball - $50.00*







RELEASE OF ALL CLAIMS

In case of an emergency you are authorized to take such measures and arrange for such medical and hospital treatment as you may deem advisable for the health and well-being of my child. I release Resurrection Baptist Church, staff, and volunteers from claim or liability due to sickness or injury.

I attest to the fact that the above named child is covered by an insurance policy covering illness and injury. I accept all financial responsibilities concerning any medical emergency. I also accept responsibility to have my child picked up immediately in the event of illness, accident, or disciplinary reasons.

In consideration for my child being allowed to participate in activities sponsored by Resurrection Baptist Church, I/we hereby release, discharge, indemnify, and agree to hold harmless Resurrection Baptist Church, its directors, officers, and employees, agents, and all volunteer personnel from any and all liability for personal injuries and/or damage(s), injury, or illness that may be suffered by. This would include any claim and/or damages it, or its agents are required to pay as a result of any injury or damage including reasonable attorney fees, litigation expenses, and court costs.
